 MEDICAID WAIVER

¨  Medically Eligible as prescribed by your doctor.

¨ Financially Eligible as determined by West Virginia SSI limits.

¨ Client must need and receive assistance with
personal care including bathing, grooming, dressing and toileting.

¨ Other assistance with mobility, meal preparation, grocery/pharmacy shopping, and light housekeeping including laundry.

¨ “Personal Care” allows up to 60 hours of in-home aid per month; the “Waiver” allows up to 155.

LIGHTHOUSE

¨ 60+ Years of Age with a Medicare Card.

¨ Medically Eligible based on a functional evaluation by a Senior Monongalians R.N.

¨ Financially Eligible determined by a sliding scale fee provided by the state of West Virginia.

¨ Client must need and receive assistance with
personal care including bathing, grooming, dressing and toileting.

¨ Other assistance with mobility, meal preparation, grocery/pharmacy shopping, and light housekeeping including laundry.

¨ Program provides up to 60 hours per month of in-home aid.

RESPITE

¨  Medically Eligible based on doctor’s diagnosis of   alzheimers / dementia.

¨ Financially Eligible determined by a sliding scale fee provided by the state of West Virginia.

¨ Program offers “sitter” type service to relieve the caregiver.  Aide will partake in social activities such as puzzles, card games, and conversation with the patient while the primary caregiver is away or
occupied elsewhere in the home.

¨ NO bathing or grooming services provided. 
Toileting and light meal preparation are provided as needed.

¨ Program provides up to 16 hours each week of respite service in the patient’s home.

PRIVATE PAY
In-home services as provided by the above programs
can be arranged at a rate of $15.50 per hour.  This is beneficial if you desire more hours than the program allows, or for some reason are deemed ineligible for the program that you desire.

Adult Day Services
Located conveniently at the
Mountaineer Mall, Senior Monongalians offers the L. June Morgan Adult Life Center.  With a loving staff, plenty of activities, a hot lunch and snacks, this day services provides YOU  with peace of mind that your senior is receiving adequate care and constant supervision while you are at work or occupied with your own activities.  HOURS: 8:00am—5:00pm, M-F

WHO CAN ENROLL?

The Adult Life Center is for people
over the age of sixty who are:
Continent, and Ambulatory (assistive devices such as walkers or canes are fine).
