Medicaid Aged & Disabled Waiver

Persons must be determined both medically and financially eligible prior to starting services.

Hours of Service Provided:
The "A/D Waiver" allows up to 155 hours of in-home aid per month
Eligibility

  • Medical eligibility is determined by a medical assessment

  • Financial Eligibility determined by West Virginia SSI limits

  • Clients must need and receive assistance with personal care such as bathing and dressing.

  • Contact your doctor for a referral

IN-HOME HEalth

Lighthouse Program

The Lighthouse Program is designed to assist those whose income or assets disqualify them from Medicaid services. The majority of the program is paid for by state funding grants to the center, however a sliding scale fee co-pay per hour of service is required and billed monthly.

Provides Support In:

  • Personal Care

  • Mobility

  • Nutrition

  • Housekeeping

Hours of Service Provided:

An individual may receive up to 60 hours per month based on need and availability of caregivers.

Eligibility:

Eligibility is determined by a Senior Monongalians Inc. Registered Nurse. Clients must be 60+ years old.

Cost:

An hourly co-pay fee is based on the household income.

FAIR: Alzheimer's Respite Program

The majority of the program is paid for by state funding grants to the center, however a sliding scale fee co-pay per hour of service is required and billed monthly.

Service Provided:

This program offers a sitter/companion type service to relieve the caregiver. The aide will encourage participation in activities such as crafts, puzzles, board and card games, and conversations. Toileting and light meal prep services are also provided.

No bathing or grooming services provided.

Hours of Service Provided:

An individual may receive up to 16 hours of care per week

Eligibility:

Medical eligibility is determined by:

  • A doctor's diagnosis of Alzheimer's or dementia

  • An in-home evaluation by our nursing staff

Cost:

An hourly co-pay fee is based on the household income.

Veteran AFFAIRS

Senior Monongalians is approved by the VA to provide a variety of in-home services.

Eligibility:

Contact your VA Caseworker to determine eligibility and initiate services.

Private Pay Care

Personal Care and Respite services are also available at a cost of $25.00 per hour when workers are available. Private Pay offers more flexibility of tasks to meet your needs best.

Hour of Service Provided:

Hours vary and have more flexibility of hours per month than other programs

Cost:

Private pay, in home care costs $25.00 per hour.
